Q: What is the Public School Live Scan Finger Print Services and why is it required?
A: The Public School Live Scan Finger Print Service is required by law for all volunteers
in the public schools. The service essentially requires you to provide a valid California
Driver’s license and your fingerprint. The service runs your fingerprint through a
Department of Justice database and verifies there is no criminal record.
Q: How much does the service cost?
A: The service costs $47. You should bring a personal check or cash. No credit cards
will be accepted at the mobile unit on site.
Q: Do I have to complete this application each year?
A: No. You only need to complete the application once and it will be valid for the time
your have children in the San Mateo-Foster City School district (K-8).
Q: How will my application information be kept private?
A: The school district and service provider have strict privacy policies that have to be
maintained by law.
Q: How do I get the application completed if I can’t make the mobile service timeslot?
A: Contact the UPS Store, at 650-598-9611 to arrange a private screening at their
facility which is located at 951-2 Old Country Road in Belmont. Be sure to pick up a
Live Scan Service finger print form (3-ply copy) in the Borel Office prior to your
scheduled appointment.

Borel Middle School
Volunteers’ Fingerprinting Requirements – Live Scan Service
Volunteers who work directly with students and are NOT in the presence of a certificated
teacher must be fingerprinted and need to follow the steps mentioned below:
1. Get a pre-printed Live Scan Service Finger print form (3 ply copy), from the Office of the
Administrative Assistant (Tania Ionoff) at the Borel school office. Complete the form schedule a private appointment at the UPS Store, 951-2 Old County Road
in Belmont.

At your appointment time go to the store with the following:
a. A $47 personal check or cash,
b. Driver’s license, and
c. Completed Live Scan Service Form (3 ply copy)
2. Return the yellow part of the completed Live Scan Service form to the Borel school office
after you have been fingerprinted.
3. Our school district will notify the school when the volunteer has been cleared to start
working with students or driving on trips. It takes at least three weeks.

do everything possible to ensure the health and safety of every child in our care.
Please NOTE:
Fingerprinting is only required ONCE from each volunteer and is maintained at the
San Mateo-Foster City School District level. The file will remain active and
applicable even if your student changes schools.
Those who drive students on field trips must still submit driver forms, driver’s
license, and insurance information annually.
