To Register Your Child You Will Need To Take The Following To Your Neighborhood School: Click on the links to access the information and forms. Note, you will need Acrobat Reader installed to view the forms.
- A completed New Student Registration Form
- A completed Home Language Survey
- A completed Informal Primary Language Survey - If Applicable: Parents who indicated English Only on the Home Language Survey do not need to complete this survey.
- A completed Report of Health Examination for School Entry - Encouraged at kindergarten, but not required until 1st grade
- A completed Affidavit of Residence for Parent and Child Sharing A Residence with Another Adult
- ORIGINAL Birth certificate or passport - Note: children enrolling in kindergarten must be five years old on or before December 2nd)
- Up-to-date Immunization Record - click here for Immunization Requirements
- Two ORIGINAL current proofs of address (i.e. utility bills, lease agreement, close of escrow papers); AND, if you reside in a domicile where your name does not appear on the deed, lease or rental agreement or other official documentation
